CagriSema matches tirzepatide on weight but misses HbA1c non-inferiority

In the 68-week open-label REIMAGINE 4 trial in adults with type 2 diabetes, CagriSema 2.4/2.4 mg was non-inferior to tirzepatide 15 mg for weight reduction but not for HbA1c reduction. Novo disclosed the result in its Q2 2026 report. The miss on blood sugar control is a setback for CagriSema's positioning in diabetes against Eli Lilly's tirzepatide.

  • Weight loss met non-inferiority versus tirzepatide 15 mg (if-all-adhered: 15.2% vs 15.8%)
  • HbA1c non-inferiority not met (drop of 1.9 vs 2.2 points on an if-all-adhered basis)
  • 68-week open-label head-to-head trial in type 2 diabetes
  • Disclosed in Novo's Q2 2026 6-K filed on 4 August 2026
